Membership fees are set up to accommodate practices of all sizes. Membership costs $250 for the first provider, $50 for the second provider, $40 for the 3rd provider, and $30 for each additional provider thereafter. Dues are a vital source of revenue for the foundation, and contribute to the administration of all of these educational programs and group opportunities.
